Kindergarten Immunization Requirements
| Doses | Vaccine | Notes |
| 4 or 5 | Dtap,Dtp,DT | The last dose of the vaccine must be given after the child's 4th birthday. |
| 3 or 4 | Polio (IPV,OPV) | The last dose must be given after the child's 4th birthday. |
| 2 | MMR | This is the combination of measles, mumps and rubella. The first dose is given on of after the 1st birthday and the second given at least 28 days later. |
| 3 | Hepatitis B | This is a series of 3 shots, usually stared at birth and given over a 6 month time period. |
| 1* | Varicella | *The chicken pox vaccine will be a requirement in the 2006 -2007 school year. |
Immunization Clinics are held at Bluffton St. Mary's Church on the second Tuesday of each month from 4:00 to 6:00 pm.
Allen County Health Department has immunization clinics on Wednesday afternoon from 1:30 - 3:30 and on Thursday
morning from 8:30 to 10:30. Evening hours are from 4:00 to 6:00 on the 2nd and 4th Thursday of the month.
*****These vaccines need to be on file at school within 2 weeks of school entry according to state law.***
